We will capture a digital image of your fingerprints and send it to the Utah Bureau of Criminal Identification for a background check.
At the request of your agent, we will conduct a blood draw, urinalysis, physical measurements, and an Electrocardiogram (EKG) to assess your health in relation to your policy.
We conduct a variety of drug and alcohol testing services for both Department of Transportation (DOT) and non-DOT requirements. Our offerings include pre-employment testing, post-accident assessments, reasonable suspicion evaluations, as well as individual and personal testing.
Our services are available both on-site and in-office. If you require a collection to be sent to an existing laboratory you currently use, we can coordinate the dispatch of the sample with the laboratory. Please call ahead of time so we can arrange this before the sample is collected. For on-site visits a $0.72 per mile fee will apply.

If a blood sample is required for testing, a qualified phlebotomist will obtain your blood following the specific requirements of the test. We collaborate with various laboratories that offer a wide range of testing options. Typically, the testing kit will be sent to you, and we will perform the blood draw, ensuring it complies with the necessary standards for return. If a urine sample is needed alongside the blood draw, an extra fee of $10 will apply.
